Service Description

A medical cannabis evaluation includes an initial medical cannabis evaluation and certification, and general usage recommendations. We can also assist with facilitating the Board of Health Professions registration paperwork so that you can receive your State Medical Cannabis Card. The price of this consultation does not include the state $50.00 application fee.

Cancellation Policy

No Show/Late Cancellation Policy: This policy has been established to help us serve you better. We must make appointments to see our patients as efficiently as possible. No-shows and late cancellations cause problems beyond a financial impact on our practice. When an appointment is made, it takes an available time slot away from another patient. No-shows and late cancellations delay the delivery of healthcare to other patients. A "no-show" is missing a scheduled appointment without calling us to cancel 24 hours before your appointment. A charge of $50.00 will be assessed for each no-show visit appointment if less than a 24-hour notice is given. Please understand that insurance companies consider this charge entirely the patient's responsibility.
