Service Description

This service is typically covered by insurance. However, there is a $50 holding fee required to secure the appointment. The fee will be maintained if a 24hr notice of cancellation is not provided and copayment or cost share fees will be applied. A cognitive evaluation includes s full review of your cognitive function, establishing or confirming a cognitive impairment, and a care plan.

Cancellation Policy

No Show/Late Cancellation Policy: This policy has been established to help us serve you better. We must make appointments to see our patients as efficiently as possible. No-shows and late cancellations cause problems beyond a financial impact on our practice. When an appointment is made, it takes an available time slot away from another patient. No-shows and late cancellations delay the delivery of healthcare to other patients. A "no-show" is missing a scheduled appointment without calling us to cancel 24 hours before your appointment. A charge of $50.00 will be assessed for each no-show visit appointment if less than a 24-hour notice is given. Please understand that insurance companies consider this charge entirely the patient's responsibility.
