In Virginia, a recommendation from a licensed, state-certified healthcare provider is required, and our providers are fully licensed and certified to issue these recommendations.
How To Start The Process
A medical marijuana evaluation involves a thorough review of medical history and symptoms by a licensed provider. The provider assesses whether a qualifying condition is present and discusses the potential benefits and risks of medical marijuana use.

Proof Of Identity

A valid driver’s license, passport, or government-issued ID is required. Patients must be Virginia residents and provide proof of residency. Additionally, an intake form must be completed prior to the visit.
Make An Appointment

All electronic intake paperwork must be completed the day before the appointment. Medical records supporting the claimed condition are required prior to the visit. Call the office for details.

Medical Cannabis Evaluation Cost
Please note that Medical Cannabis Evaluations are not covered by insurance. All charges below include an evaluation for treatment appropriateness and an electronic certification.​

$180 for a medical cannabis consultation and electronic submission of the recommendation to the Board of Health Professions for an electronic certification.

$100 for a re- evaluation, including an evaluation and electronic certification.
