We accept most major insurance plans for both in-person and telehealth visits. If you are unsure whether your plan is accepted, our billing team is happy to verify your coverage before your visit. At Be Healthy & Well, we strive to make healthcare accessible and transparent. This policy explains the responsibility of patients for payments when insurance does not cover services, and how we handle non-payment situations. Patient Responsibility Patients are responsible for: • Providing accurate insurance information at the time of service. • Paying any copays, deductibles, or coinsurance as required by their plan. • Covering the cost of services not covered by their insurance, including telehealth visits, preventive screenings, and specialized procedures. • Promptly informing the clinic of any changes in insurance coverage. While we submit claims to your insurance, it is ultimately the patient’s responsibility to ensure payment for all services rendered. Non-Payment by Insurance If your insurance company denies a claim or fails to pay: • The patient will be responsible for the remaining balance. • Our billing team will provide an itemized statement and assist with appealing denied claims when possible. • Outstanding balances should be paid promptly to avoid interruptions in care or additional fees. Payment Expectations • Copays and self-pay charges are collected at the time of service. • Patients may arrange payment plans with our billing team if needed. • Unpaid balances may result in delayed scheduling of future appointments until the account is resolved. Returned / Declined Payment Policy Payments that are returned, declined, or reversed may result in additional fees. • Patients are responsible for ensuring a valid payment method is on file. • A returned check or declined card may result in an administrative fee. • Outstanding balances must be resolved promptly to avoid delays in future scheduling. • Continued payment issues may require prepayment for future services. Refund Policy Be Healthy & Well strives to maintain fair and transparent billing practices. • Overpayments will be refunded once verified. • No-show and late cancellation fees are non-refundable. • Refunds are not issued for services already rendered. • Approved refunds are processed using the original payment method when possible. Telehealth Billing Policy Telehealth services are billed similarly to in-person visits; however, insurance coverage may vary. • Insurance reimbursement for telehealth depends on your individual plan. • Copays, deductibles, and coinsurance may apply. • If telehealth services are not covered, the patient is responsible for payment. • Self-pay telehealth options are available upon request. Preventive vs. Problem-Based Visit Policy Preventive visits and problem-focused visits are billed differently by insurance providers. • Preventive visits typically include wellness exams and routine screenings. • Addressing new or ongoing medical concerns during a preventive visit may result in additional charges. • Insurance determines coverage and payment—not the medical provider. • Patients are responsible for charges not covered by insurance. Referrals & Prior Authorization Policy Some insurance plans require referrals or prior authorization for services. • Patients are responsible for obtaining required referrals or authorizations. • Failure to obtain authorization may result in denied claims. • Denied claims become the patient’s financial responsibility. Collections Policy To maintain accessible care for all patients: • Accounts with unpaid balances may receive billing statements and reminders. • Balances not resolved within a reasonable timeframe may be sent to collections. • Patients will be notified prior to any collections action. • Collection activity may impact the ability to schedule future appointments. Financial Responsibility Acknowledgment By receiving services at Be Healthy & Well, patients acknowledge and agree: • To pay all charges not covered by insurance. • To comply with billing, payment, and cancellation policies. • That financial responsibility applies to both in-person and telehealth services.

At Be Healthy & Well, we value your time and the time of our providers. Missed and late-cancelled appointments limit our ability to offer timely care to other patients. This policy outlines our expectations and associated fees to ensure efficient scheduling and access to care. Definition of a No-Show A no-show is defined as: • Failure to attend a scheduled appointment without prior notice, or • Cancellation with less than 24 hours’ notice, or • Logging into a telehealth visit more than 10 minutes late without communication No-Show & Late Cancellation Fee • A $50 no-show or late cancellation fee will be charged for missed appointments. • This fee applies to both in-person and telehealth visits. • No-show fees are not covered by insurance and are the patient’s responsibility. • Fees are charged to the payment method on file. Late Arrival Policy • Patients arriving more than 10 minutes late may be asked to reschedule. • If a visit proceeds, it may be shortened at the provider’s discretion. • Late arrivals may still be subject to the $50 fee. Repeat No-Shows To maintain availability for all patients: • Repeated no-shows may require prepayment for future appointments. • Continued missed appointments may result in limited scheduling availability or dismissal from the practice, in accordance with clinic policy. Emergency Consideration We understand that emergencies happen. • Fees may be reviewed on a case-by-case basis for documented emergencies. • Requests for fee review must be submitted within a reasonable timeframe. Patient Acknowledgment By scheduling an appointment with Be Healthy & Well, patients acknowledge and agree: • To provide at least 24 hours’ notice for cancellations or rescheduling. • To pay the $50 no-show or late cancellation fee if applicable. • That this policy applies to all appointment types, including telehealth.
