Crisis Stabilization Asessessment
At Be Healthy & Well, we are dedicated to providing equal access to high-quality care. We are committed to taking a holistic approach to mental health by integrating it into our services. This is why we work closely with crisis stabilization facilities to eliminate barriers and improve access to healthcare services. Our aim is to support more individuals in taking the initial step towards their mental wellness journey.
Our Providers
Our Nurse Practitioners play a crucial role in conducting crisis assessments as part of a comprehensive short-term evaluation and stabilization process for individuals who are currently going through a mental health crisis. This involves a thorough and compassionate evaluation of the individual's mental and emotional state to provide immediate support and intervention.
About Our Crisis Stabilization Assessment Services
1
Purpose
-
Crisis Stabilization: To assess and stabilize the individual’s mental health condition in a safe environment.
-
Comprehensive Evaluation: To gather information on the individual's mental health history, current symptoms, and any immediate risks.
2
Process
Initial Assessment: Upon arrival, the NP conducts a thorough evaluation, including mental status exams, risk assessments (for self-harm or harm to others), and gathering of medical history. Crisis Intervention: Immediate interventions may be implemented to address acute symptoms, which could include medication management, therapeutic techniques, or de-escalation strategies. Observation and Monitoring: The individual is monitored for 23 hours to observe changes in their mental state and response to interventions. Collaboration: The NP collaborates with other healthcare professionals to ensure a holistic approach to care, possibly involving psychiatrists, psychologists, and social workers. Discharge Planning: Towards the end of the 23-hour period, the NP develops a discharge plan, which may include referrals to outpatient services, recommendations for follow-up care, and support resources. Documentation: Comprehensive documentation of assessments, interventions, and follow-up plans is crucial for continuity of care.
3
Outcome
The purpose of a 23-hour crisis assessment is to stabilize the individual and link them with suitable ongoing treatment to prevent future crises. This process helps ensure safety, offers immediate support, and establishes the foundation for long-term mental health care.
