How to Start the Process
01 Proof of Identity

A driver's license, passport, or government-issued identification is required.

Must be a resident of Virginia, and proof of residency is required. More Details

We also require that you complete an intake form before your visit.

02 Qualifying Condition

There is a non-refundable $25 charge at the time of booking.

All electronic intake paperwork is required the day before your appointment.

03 An Evaluation

A registered provider
will review your case.

Depending on the assessment, registration with the Virginia Department of Health Professionals may be required. The fee is $50 annually.

Re-evaluations are done yearly and require a follow-up appointment to be issued an up to date certification.

The Board of Pharmacy processes applications within 7-10 business days. Once processed and issued, registration cards will be mailed to the address provided on the application.

Medical Cannabis Examination Package

$130 for a medical cannabis consultation and recommendation.

$155 for medical cannabis consultation, recommendation, and assistance with the registration paperwork for the Board of Health Professions so that you can receive your State Medical Cannabis Card.
